This tells us how to walk you through the rest of this form, and helps our system read your file's questions correctly.
2. Consent to process your data
MapleTools processes the file(s) you upload solely to generate your analysis. Raw survey responses are never published; only structural summaries are. See the Privacy notice at the bottom of this page.
Consent is required to continue - we can't process a file without it.
3. What are you uploading?
Our system needs SurveyMonkey's raw export - every response in one file, with the original two-row question header intact.
4. Do you have a copy of the blank survey questionnaire?
Optional, but it lets us reconcile your data against the original design and recover answer options nobody chose - impossible from the data alone.
5. Where should we send your results?
You'll get a confirmation now, and a results email with download links when your analysis is ready.

an English or French raw SurveyMonkey export (CSV or XLSX, all responses in one file, original two-row header intact) plus the questionnaire PDF downloaded directly from SurveyMonkey.
